ENROLLMENT AND EMERGENCY INFORMATION
It is important for us to reach you if your child is ill or injured at school. Please complete the Emergency Health Form from school, listing parent/guardian name with work, home, and cell phone numbers, along with two emergency contacts. (Please obtain permission from alternate contact people before placing their name on the card). Also, please indicate any medical concerns the school should know about. Call your child’s school with any changes throughout the school year.
